Nurse II (Job Order – Php 32, 057. 55)

Medical Technologist II (Job Order – Php 32, 057.55)

Midwife II (Job Order – Php 21,791.70)

Qualifications:

  • Registered Professional (RA 1080);
  • With atleast 1 year of relevant experience in the field of primary care; and
  • With atleast 4 hours of relevant training.

Note: Qualified applicants are required to register online through this LINK. Those who successfully registered online shall then submit the following documents to the Health Human Resource Development Unit, DOH RHTC, Osmeña Blvd., Sambag II, Cebu City or the Provincial DOH Offices in Bohol, Negros Oriental and Siquijor not later than 10 January 2020;

Nurse II (Job Order)

Accepted applicants are required to submit the following documents to the Health Human Resource Development Unit Office, DOH Cmpd., Osmeña Blvd., Cebu City or to their respective Provincial DOH Offices on or before 28 June 2019;

  1. Three (3) sets of accomplished and notarized Personal Data Sheet (PDS)               *Attached with three (3) sets of Work Experience Sheet
  2. Three (3) sets of original copy of Authenticated Certificate of PRC ID/ License 
  3. NBI Clearance
  4. CS Form No. 211, revised 2018 Medical Certificate (CBC, Urinalysis, Chest X-Ray, Drug Test)                                 
        *To be signed by a government physician 
  5. Two (2) photocopies of TOR
  6. Training Certificates
  7. One (1) Long Folder (Unlabeled)

Applicants who have complied with the employment requirements shall report to their respective areas of assignment on 01 July 2019.
