Newly Hired HRH Deployment Project 2019

Below are the accepted Human Resources for Health for Central Visayas (June- December 2019 Contract Period).
Accepted applicants are required to submit the following requirements on or before 30 May 2019:

A. Three (3) Sets of Accomplished and Notarized Personal Data Sheet (PDS)
a. Three (3) sets of Work Experience Sheet
B. Two (2) copies of Notarized SALN
C. Three (3) Sets of Original Copy of Authenticated Certificate of PRC ID/ License
D. NBI Clearance
E. Medical Certificate (CBC, Urinalysis, Chest X-Ray, Drug Test)
a. To be signed by a government physician
F. Two (2) Copies of TOR
G. Training Certificates
H. Pag-ibig Enrollment Form with MID No.
I. PSA Birth Certificate (Photocopy)
a. Marriage Certificate for Married (Photocopy)
J. One (1) Long Folder (Do Not Label)

ADVISORY: STATUS OF DOH HRH DEPLOYED PERSONNEL

The services of the DOH Human Resources for Health (HRH) Deployed Personnel hired last February 2019 shall continue for the 2nd Quarter of 2019, until further notice is given. The Contract of Service (COS) is three (3) months which shall commence on the 1st day of April 2019 and end on the last working day of June 2019. Conversely, they are to report at their designated areas of assignment for the duration of the contract.

The salaries of said HRH will now follow the Salary Standardization (4th Tranche) set by law, including other adjustments incidental therefor. However, pending the enactment of the 2019 General Appropriations Act and issuances of the corresponding guidelines, the additional hiring of HRH personnel will be temporarily put on hold.
