Nurse II (Job Order)
Accepted applicants are required to submit the following documents to the Health Human Resource Development Unit Office, DOH Cmpd., Osmeña Blvd., Cebu City or to their respective Provincial DOH Offices on or before 28 June 2019;
- Three (3) sets of accomplished and notarized Personal Data Sheet (PDS) *Attached with three (3) sets of Work Experience Sheet
- Three (3) sets of original copy of Authenticated Certificate of PRC ID/ License
- NBI Clearance
- CS Form No. 211, revised 2018 Medical Certificate (CBC, Urinalysis, Chest X-Ray, Drug Test)
*To be signed by a government physician - Two (2) photocopies of TOR
- Training Certificates
- One (1) Long Folder (Unlabeled)
Applicants who have complied with the employment requirements shall report to their respective areas of assignment on 01 July 2019.

ADVISORY: STATUS OF DOH HRH DEPLOYED PERSONNEL
The services of the DOH Human Resources for Health (HRH) Deployed Personnel hired last February 2019 shall continue for the 2nd Quarter of 2019, until further notice is given. The Contract of Service (COS) is three (3) months which shall commence on the 1st day of April 2019 and end on the last working day of June 2019. Conversely, they are to report at their designated areas of assignment for the duration of the contract.
The salaries of said HRH will now follow the Salary Standardization (4th Tranche) set by law, including other adjustments incidental therefor. However, pending the enactment of the 2019 General Appropriations Act and issuances of the corresponding guidelines, the additional hiring of HRH personnel will be temporarily put on hold.
