Below are the accepted applicants under the 2017 HUMAN RESOURCES FOR HEALTH of CENTRAL VISAYAS.
Accepted applicants are instructed to proceed to their assigned Rural Health Units on January 30, 2017 for their first day of work. Furthermore, newly-hired personnel including those that are occupying new positions are enjoined to visit the Department of Health - Regional Office or any Provincial DOH Office and submit the following requirements on or before February 17, 2017.
A. Accomplished Personal Data Sheet (PDS)
B. Diploma (photocopy)
D. CSC Form No. 211- Medical Certificate signed by Government Physician (Blood Test, Urinalysis, Chest X-ray and Drug Test)
E.NBI Clearance
F. One (1) Long Brown Envelope
G. If without Tax Identification Number (TIN) please secure and accomplish BIR Form 1902.

Reminders for the Human Resources For Health (HRH) Qualifying Examinations for the Nurse Deployment Project, Public Health Associate Deployment Project, Universal Health Care Implementer Deployment Project and Rural Health Midwives Placement Program
- Only applicants listed below are eligible to take the examinations
- Schedule of examination shall be on January 9-11, 2017. Applicants are advised to come on their designated schedule below. Tardy applicants will not be entertained.
- Venue of examination will be announced later
- Applicants are strictly advised to bring the following:
- Black pen
- Valid PRC ID. Incase of expired PRC IDs, kindly bring your PRC Claim stub.
- Another valid government issued ID with picture for verification purposes.
- Mobile phones, electronic devices and bulky bags are not allowed in the examination room. These items shall be left outside the examination room.
- Applicants are encouraged to wear decent clothing during the examination. Slippers, mini-skirts and spaghetti straps are not allowed in the examination room. (Please click read more to view list of examinees)
